Colorado’s Medicaid program is called Health First Colorado. Every Health First Colorado member is assigned a primary care medical provider (PCMP) and a regional accountable entity (RAE), also called a regional organization. A RAE helps you manage your physical and behavioral health benefits and connects you to the care and resources you need. Your RAE is assigned based on the location of your PCMP and is listed on your Health First Colorado enrollment letter. You can change your PCMP at any time, and if you do, your RAE may change.
Rocky Mountain Health Plans, a UnitedHealthcare company, serves as the RAE for Region 1 in Colorado.
RAEs are responsible for coordinating a member’s care. They support a network of providers to make sure you can access physical health care, behavioral health care, vision and dental services, and health-related social need resources.
As the Region 1 RAE, Rocky Mountain Health Plans works together with your PCMP, community care teams, and other local partners to help you get the care you need, when and where you need it.

RMHP brings members, families, and community partners together through advisory councils and committees that help improve Medicaid and CHP+ programs. These groups elevate member perspectives, inform program and policy decisions, and address health equity across the regions we serve. Meetings are held regularly, with options to participate in person or remotely. We welcome diverse voices and support participation through reimbursed travel when applicable. All members assigned to our regional accountable entity (RAE), sometimes called regional organization, receive the same benefits and services that are covered under Health First Colorado.
