Northeast Health Partners

NHP serves as the Regional Accountable Entity (RAE) for Region 2, and RMHP operates as a subcontracted Administrative Services Organization (ASO). RMHP supports NHP in delivering key behavioral health functions including network development, contracting, and credentialing.

Colorado LIFTS (Linking Individuals and Families to Services) connects people in Colorado to affordable mental health, substance use, and crisis care. LIFTS is dedicated to providing the highest quality care to all people of Colorado, regardless of insurance.

  • Behavioral Health & Crisis Support
      • The 988 Colorado Mental Health Line is part of a national network of local contact centers offering free and confidential assistance to people struggling with emotional, mental health, or substance use concerns. Dial or text 988, or live chat 24/7/365.
      • Care coordination for substance use is available to assist with navigating substance use treatment, resources, and recovery support. Email bhaso.carecoordination@uhc.com to connect with a care coordinator.
      • Tough as a Mother offers recovery support for parenting and pregnant mothers who are affected by substance use disorders.
  • Connect for Health Colorado
      • Connect for Health Colorado is the state’s health insurance marketplace for individual and family health plans. If you do not qualify for Health First Colorado (Colorado’s Medicaid Program), you may still be eligible for financial assistance to lower the cost of your monthly premiums.
  • Quit Tobacco
      • Baby & Me Tobacco Free is a program that helps pregnant women quit smoking and offers vouchers for diapers and baby wipes for women who test tobacco free during pregnancy and after the baby is born.
      • The Colorado Quitline allows you to talk, text, or chat with a quit coach to support your journey to becoming tobacco free. Call 1-800-QUIT-NOW to get started.
  • Care Coordination
      • We offer comprehensive care coordination services designed to promote access to healthcare and community resources that help you achieve your health and wellness goals.

  • Food Assistance
  • Health First Colorado (Colorado’s Medicaid Program)
      • Health First Colorado provides health insurance benefits to people who qualify. Learn more about the program, your benefits, how to apply, and the renewal process on their website.
      • You can apply for or manage your state benefits, including SNAP, through Colorado PEAK.
  • Case Management Agency
      • We proudly serve as the case management agency (CMA) for designated service areas 13, 14, 16, 17, and 18. As the CMA, we help you find the quality care that’s right for your individual needs while helping you maintain your independence as much as possible. Visit our CMA page to learn more about the program and to find contact information for your area. 
      • Long-Term Services & Supports (LTSS) are programs that can help people who have disabilities or older adults with their daily activities.
