How the Hidden Work of Case Management Helps Prevent Hospital Stays and Readmissions

If you’re managing a chronic condition, recovering from a hospital stay, or balancing multiple health and daily living needs, managing your health care can feel overwhelming. You may have multiple providers, medications, appointments, and services to track, all while trying to stay focused on your health.

That’s where case management comes in.

What is Case Management?

Case management is designed for people with complex or ongoing medical needs, including patients living with chronic conditions, disabilities, or living situations that make it more difficult to access and manage care. It helps ensure you have the support, guidance, and resources needed to stay well and avoid unnecessary hospital visits.

A case manager works with you to coordinate your care, connect you to services, and guide you through the health care system. Just as importantly, case managers are your advocates. They support you, ensure your voice is heard, and help you make informed decisions about your care.

The Hidden Work That Makes a Difference

Much of what case managers do happens behind the scenes, but it can have a big impact on your health. Case management has been shown to reduce hospital stays, emergency room visits, and readmissions, while improving quality of life and lowering health care costs.

In this role, we help individuals and families access long-term services and supports, coordinate care, and navigate available resources.
