By aligning provider payments with quality, prevention, and access—particularly in rural and underserved communities—the PRIME program supports more sustainable care delivery across Colorado. As a value-based care model for Medicaid members, PRIME emphasizes whole-person, coordinated care to improve health outcomes statewide. The program has demonstrated measurable improvements while reinvesting savings back into local health systems and communities.

Colorado's Behavioral Health Administration launched the Linking Individuals and Families To Services (LIFTS) initiative, aimed at connecting Coloradans to a statewide, connected network of mental health, substance use, crisis support, and care navigation — regardless of insurance coverage status. Rocky Mountain Health Plans proudly serves as the BHA Service Organization for Region 1 and is honored to help launch the Colorado LIFTS network.

Meaningful change starts with listening. Through weekly planning meetings, Rocky Mountain Health Plans, Pueblo Azul, Oliver Behavioral Consultants, and MAS Color have collaborated to better understand the pressing disparities facing Spanish-speaking families in the Roaring Fork Valley and taken action to support those with complex medical needs.

Silt parent Indhira Barrón started the peer-support and resource group Pueblo Azul Colorado for Spanish-speaking parents and caregivers of children with disabilities. Rocky Mountain Health Plans was proud to attend a recent meeting to share information about Medicaid and health coverage for kids and young adults with disabilities.
