Individual and family plans are available through private insurers or health insurance marketplaces, such as Connect for Health Colorado. They can vary in premium and out-of-pocket costs, benefits and covered services, and provider networks. Subsidies are available for those who qualify to help lower your monthly premium.
Medicare is health insurance for people age 65 and older, as well as certain eligible individuals with disabilities or specific medical conditions. Types of Medicare plans include Original Medicare, Medicare Advantage, Medicare supplement, and prescription drug plans.
Dual special needs plans (D-SNPs) are Medicare Advantage plans for individuals who qualify for both Medicare and Medicaid (also called dual eligible members). It is a Medicare Advantage plan that offers more benefits and features than Original Medicare and allows you to keep your Health First Colorado benefits. Learn more about D-SNPs.


Colorado’s Medicaid program is called Health First Colorado. It is free or low-cost health care for Colorado children and adults who meet certain income requirements. Rocky Mountain Health Plans also offers PRIME, a program for adults and some children with disabilities who live in select Western Slope counties and meet specific eligibility requirements.
Child Health Plan Plus (CHP+) is low-cost health insurance for eligible children and pregnant women who do not qualify for Health First Colorado but don’t earn enough to pay for private health insurance.
