About RMHP Prime

RMHP has a health plan called RMHP Prime that provides healthcare services to most adult Health First Colorado Members and some children with disabilities in certain counties on the Western Slope:  Delta, Garfield, Gunnison, Mesa, Montrose, Ouray, Pitkin, Rio Blanco and San Miguel.

Getting Care with RMHP Prime

When you are enrolled in RMHP Prime, most of your Health First Colorado benefits are covered by RMHP. Through a community-based approach, RMHP provides benefits and care coordination with a focus on integration of behavioral and physical health services.

• You'll be connected to providers, as well as community and social services.
• We can help you get the care you need after a stay in the hospital or nursing facility.
• We can help you choose a Primary Care Provider who will serve as your medical home to help you get the most out of your healthcare, including coordinating with specialists, should you need one.
