About the Regional Organization

RMHP is the regional organization for Health First Colorado (Colorado's Medicaid Program) Members in Western Colorado and Larimer County. We also serve some Members through RMHP Prime. We help coordinate your care and make sure you get the help you need, when you need it.

Every Health First Colorado (Colorado's Medicaid Program) Member belongs to a regional organization. RMHP is the regional organization for Region 1. We help Health First Colorado Members in Western Colorado and Larimer County access their benefits and live healthier lives. RMHP works together with your primary care provider, community care teams, and other local partners to help you get the care you need. We will:

• Assess your needs
• Refer you to people who can help
• Work together with you toward your health and wellness goals

Together, we will work with you and with any other providers who care for you to:

• Coordinate the different services you get
• Help when your health care needs change
• Help you learn about caring for yourself
