Has your contact information changed? Have you moved in the past three years?
Make sure that Health First Colorado (Colorado’s Medicaid program) or Child Health Plan Plus (CHP+) have your current phone number, email and mailing address. It is important that you can be contacted if you need to fill out important paperwork.

There may be steps you need to take to renew your coverage. You may be sent a renewal packet or an information verification letter to sign and return. This can be done by mail, on a computer or over the phone.
You can update your information in one of these ways:
- Visit Colorado.gov/PEAK. If you don’t have a PEAK account, you can create one at Colorado.gov/PEAK.
- Use the Health First Colorado app on your phone. This free app is for Health First Colorado and CHP+ members. Download it for free in the Google Play or Apple App stores.
- Medicaid and CHP+ members can call 800-359-1991 (State Relay: 711). Help is available in multiple languages.
- Contact your county department of human services.
- Contact your local application assistance site.

Coordinated Care in the Community
RMHP was recently named the Single Entry Point (SEP) agency in Mesa County. Our goal as the SEP is to help our members find the care that’s right for their individual needs while helping them maintain their independence as much as possible. As the SEP, we will determine if a member is eligible for long-term care services, like home- and community-based services, long-term home health, nursing home care, and home care allowance. We will then serve as the point of contact between programs and members in need of this care.
