Masters 1 and 2

In the Masters Level 1 & 2 programs, your practice will build and utilize skills and infrastructure that are consistent with a comprehensive primary care approach. Practices will focus on and expand upon care management of high risk patients and coordination of care across the medical neighborhood.


Masters Level 1

Participation in the Masters 1 Program helps your practice integrate key concepts and services offered in advanced primary care. This program allows for the appropriate time to test, prepare for, and eventually implement payment reform opportunities. Specific attention and action in the program focuses on the three components of the Institute for Healthcare Improvement (IHI)’s Triple Aim, as well as Bodenheimer’s and Sinsky’s Fourth Aim: provider satisfaction. Collectively, with research from both the Institute for Healthcare Improvement and Bodenheimer & Sinsky, healthcare organizations all over the country, including Rocky Mountain Health Plans, are pursuing what we now call the Quadruple Aim:

  • Improve the health of the population
  • Enhance the patient experience of care (including quality, access, and reliability)
  • Control the per capita cost of care
  • Improve provider/clinician job satisfaction

Program Objectives

  1. Creating a strong foundation for population management
  2. Managing services at scale for defined populations
  3. Establishing a learning system to drive and sustain work
