Because practice transformation enhances the effectiveness of patient care and population health, RMHP incentivizes practices to participate in these programs. Twice each year, we actively recruit primary care practices to participate in our program.
Programs
Foundations PCP
Foundations is an introductory level course in which participants learn basic QI skills at the practice level to include Plan, Do, Study, Act (PDSA) cycles, process mapping, and basic data.
Masters 1 & 2
In the Masters programs, your practice will build, utilize, and enhance skills and infrastructure that are consistent with a comprehensive primary care approach.
PCMH
The Patient-Centered Medical Home (PCMH) is a model of care that emphasizes care coordination and communication to transform primary care into "what patients want it to be."
PCSP
The Patient-Centered Specialty Practice (PCSP) is an NCQA program focusing on specialty practices that are committed to access, communication, and care coordination.
CO-EARTH & SIM
Colorado’s State Innovation Model (SIM) and the CO-EARTH program both focus on improving coordination, collaboration and integration of behavioral health and primary care services.
TCPI
The Transforming Clinical Practices Initiative (TCPI) is a free, federally-funded support network that will help you and your care team prepare for value-based payment.

CPC+ is a partnership between payer partners from CMS, state Health First Colorado agencies, commercial health plans, self-insured businesses, and primary care providers.
