Rocky Mountain Valley plans include a high-performing network of providers and facilities across the Western Slope. The plans offer access to an extensive network of local Western Slope providers, all Denver-area physicians and facilities, and our broad pharmacy network. Rocky Mountain Valley plans are available to residents in Archuleta, Dolores, Eagle (Basalt), Garfield, Gunnison, Hinsdale, La Plata, Montezuma, Montrose, Ouray, Pitkin, Routt, San Juan, and San Miguel counties.

Monument ONE: Available in Mesa, Moffat and Rio Blanco Counties
Monument ONE plans provide access to care from providers you know and trust, right here in your community. Including, University Hospital, Children’s Hospital, and all SCL Health providers and facilities in Denver and surrounding counties. Emergency care is always covered as in-network.
Monument Health HMO: Offered in Mesa, Delta, Moffat and Rio Blanco Counties
Monument Health HMO plans are offered for individuals & families in Mesa, Delta, Moffat and Rio Blanco counties. The tiered networks allow you to choose the provider and coverage that’s right for you, allowing access to the Monument Health Network (Tier 1), as well as other select providers across the Western Slope and in Denver and Colorado Springs (Tier 2). Emergency care is always in-network.
