A Regional HMO, or Health Maintenance Organizations, offers access to a select group of providers. As a Member of a regional HMO plan, it is important to select a primary care provider (PCP) from a network of local healthcare providers who will refer you to in-network specialists or hospitals when necessary. Your PCP will help coordinate your care

RMHP’s Monument Health HMO plans are offered and centered around where you and your family seek care – close to home. While we encourage you to select a PCP, you are not required to get a referral to see any provider in the Monument Health HMO network.

These plans are available to Mesa County residents and offer comprehensive coverage via a two-tiered benefit structure, allowing for access to the Monument Health Network (Tier 1), as well as other select providers across the Western Slope and in Denver and Colorado Springs (Tier 2). Emergency care is always in-network. Monument Health’s high-performing clinically-integrated network offers Members local access to high-quality, coordinated health care with a vision of a healthier Western Colorado. The Open Enrollment Period for 2019 Individual & Family Plans is November 1, 2018 - January 15, 2019.
