What is an HMO?
A regional HMO, or Health Maintenance Organization, offers access to a select group of providers. As a Member of a regional HMO plan, it's important to select a primary care provider (PCP) from a network of local healthcare providers who will refer you to in-network specialists or hospitals when necessary. Your PCP will help coordinate your care.

RMHP’s Monument Health HMO plans are offered and centered around where you and your family seek care – close to home. Monument Health’s high-performing clinically-integrated network offers Members local access to high-quality, coordinated health care with a vision of a healthier Western Colorado. While we encourage you to select a PCP, you are not required to get a referral to see any provider in the Monument Health HMO network.
About Our HMO Plans
New for 2020: Monument ONE
RMHP is pleased to offer our new Monument ONE product for individuals & families in Mesa County. This single-tier plan allows you to get care from providers you know and trust, right here in your community. The network includes six primary care practices (more than 125 primary care providers) to serve as your medical home; two local hospitals (St. Mary’s Medical Center and Colorado Canyons Hospital & Medical Center); many local specialists who are either independently owned or affiliated with St. Mary’s Medical Center or Family Health West; and University Hospital, Children’s Hospital, and all SCL Health providers and facilities in Denver and surrounding counties. Emergency care is always covered as in-network.
Monument Health HMO: Now Offered in Delta County
Monument Health HMO plans are now available for individuals & families in Mesa and Delta counties. The tiered networks allow you to choose the provider and coverage that’s right for you, allowing access to the Monument Health Network (Tier 1), as well as other select providers across the Western Slope and in Denver and Colorado Springs (Tier 2). Emergency care is always in-network.

The Open Enrollment Period for 2020 Individual & Family Plans is November 1, 2019 - January 15, 2020. Our plans are available directly through RMHP by contacting our dedicated Individual Sales team, or by getting in touch with your local broker. Plans are also available through Connect for Health Colorado, the state’s health insurance marketplace. You may be eligible for Federal financial assistance if you enroll through Connect for Health Colorado.
