Chronic Care Management
In 2006, Rocky Mountain Health Plans received national recognition from the American Association of Health Plans (AHIP) in the area of chronic care. The award, named the 2006 AHIP Foundation Innovation and Excellence Award: Chronic Care, was given in acknowledgement of the Rocky Mountain Health Plans chronic care program, "Improving Diabetes Outcomes in Mesa County, Colorado".
Designed and implemented by the Rocky Mountain Health Plans Quality Improvement Department, the program approach combines physician pay-for-performance with system redesign in the primary care physician office to improve the health outcomes for Members with diabetes. The program has resulted in statistically significant improvement in the outcomes for the entire diabetic population of nearly 2,000 Members in the Mesa County area. This program has made a difference in the lives of Rocky Mountain Health Plans Members and for other diabetics in the Mesa County community, by creating a model that will be implemented in communities throughout Colorado.
Additionally in the area of chronic care management, Rocky Mountain Health Plans employee, Nancy Mohler, RD, MS, CDE, chronic disease population specialist, received the 2006 national award for Outstanding Diabetes Educator of the Year. This award was presented to Mohler by the American Dietetic Association, a national organization of more than 65,000 professional dietitians. Mohler, a registered dietitian and certified diabetes educator, supports Rocky Mountain Health Plans in providing physician education on quality care for chronic illness throughout Colorado.
Well Workplace
Rocky Mountain Health Plans has been named a Well Workplace by the Wellness Councils of America. RMHP earned this recognition by successfully meeting rigorous health promotion standards and by demonstrating a commitment to improving the health and well being of its employees. This prestigious acknowledgement recognizes RMHP as one of America's Healthiest Companies. Rocky Mountain Health Plans is the first employer on the Western Slope to receive this award and the fifth in Colorado.
2005 Outstanding Corporate Partner, Juvenile Diabetes Research Foundation
Rocky Mountain Health Plans was the proud recipient of the 2005 Outstanding Corporate Partner Award from the Rocky Mountain Chapter of the Juvenile Diabetes Research Foundation (JDRF). This acknowledgement is given with great care and consideration to a company that demonstrates a high level of support to JDRF and its unrelenting mission to find a cure for Type 1 diabetes. Top Company 2005 and 2006 Finalist, Colorado's Most Competitive Business Award
For the past two years, 2005 and 2006, Rocky Mountain Health Plans has been honored by ColoradoBiz magazine as a finalist for Top Company in the health care category. The ColoradoBiz Top Company awards program was established in 1988 to recognize Colorado companies and to showcase those businesses whose financial and operational excellence and commitment to the community set the standard for those in business. Finalists are selected based on financial performance, excellence in one or more aspects of their business and community involvement. RMHP is proud to be part of this prestigious business list.
Advertising Excellence Davey Awards
Rocky Mountain Health Plans and its advertising agency, COBB and Associates, are recipients of two Davey Awards for the 'Good health. That's the plan.' advertising campaign. The awards are in the categories of Best Not-for-Profit TV Commercial and Best Use of Animation in a TV Commercial.
The Davey Awards are named after David of Biblical Times. David defeated the mighty Goliath with a big idea and a little rock. The Davey Awards program is overseen by the International Academy of the Visual Arts, ADWEEK magazine and Fortune Small Business magazine.
Colorado Healthcare Communicators Association
Rocky Mountain Health Plans won a 2005 Silver Leaf Award for Best Print Advertising Campaign, a 2006 Silver Leaf Award for Best Communications Program, Internal and External and a 2006 Bronze Leaf Award for Best Print Advertising campaign.
Colorado Healthcare Communicators is a professional organization with a mission to serve its members by raising the standards of professional knowledge and performance; furthering professional development and interaction among healthcare communications professionals; and expanding the role of members in management.
Website Design
The Rocky Mountain Health Plans website, www.rmhp.org, has earned a 2006 Healthcare Leadership Award for Best Site Design. RMHP competed on a national level with other large, well-known carriers. The award is presented by the publication eHealthcare Strategy & Trends.
