Change My PCP

To change your PCP of record, complete the Change MY PCP form then submit it on-line. Each covered member of your family may select a different PCP.  To locate a provider in your area, use the Provider Directory.  You can also contact Customer Service for a list of providers in your area.  Call Customer Service 8:00 a.m. to 8:00 p.m., Mountain Time, (from Oct. 1-Feb. 14, 7 days/week; from Feb. 15-Sept. 30, M-F) at 970-244-7912 or 888-282-1420 (TTY: 711). Finally, you can change your PCP on-line through access|RMHP, our secure Member website. However you submit a PCP change, be sure your new doctor’s office is accepting new patients first. You must tell us about your change of PCP before you see them. You should arrange to have your medical records transferred from your previous PCP to your new PCP.
