How to Use Your CHP+ Plan

CHP+ helps families who cannot afford health insurance and who do not qualify for Health First Colorado. CHP+ enrolls children in a health benefits plan at a low cost.
When you are enrolled in CHP+, your children go to the doctor or get medication whenever they need it. You can take your child to any of the RMHP participating providers who practice in the 22 counties of Western Colorado which are shown in the map below.
Using RMHP Services
A participating provider is a health care provider who has a contract with RMHP to serve RMHP Members. To be covered, you must get your care from a participating provider, unless you have a medical emergency; then call 911 or go to the nearest emergency room.
Note: You can see any family planning provider for family planning services even if the provider does not work with us.
Health care services covered by CHP+ include:
Choosing a PCP
- Check-ups and shots
- Well-child care and immunizations
- Medical office visits
- Teen services
- Hospital services including outpatient services, inpatient care, and inpatient doctors' care
- Glasses and hearing aids
- Mental health care, such as counseling, hospitalization, and outpatient substance abuse treatments.
Your health care is managed by a primary care physician (PCP). Your PCP must be an RMHP participating provider. Each covered member of your family can pick a different PCP. You can see a list of doctors in the Provider Directory. Be sure the doctor you pick is accepting new patients. If you have questions about which doctors you can see, call RMHP Customer Service at 970-244-7960 or 855-830-1563.
You do not need a referral (approval) to see a participating provider for covered health care services. If the health care you need is covered by CHP+ but cannot be provided by an RMHP participating provider, your PCP will work with RMHP to get you in to see the doctor you need, even if that doctor is not an RMHP participating provider.
If you are not happy about something RMHP has done, you may appeal, or ask us to look at something again. If you are not happy about something an RMHP participating provider has done, you have the right to appeal.
