Many Seniors live active and healthy lives, but there's no getting around the fact that as we age, our bodies and minds experience change. There are things you can do to stay healthy and active as you age. It is important to understand what to expect. Having a healthy lifestyle can help you to deal with normal aging changes and make the most of your life. You may also have a chronic condition that you want to learn how to more effectively manage.

Disease and Case Management Programs
Rocky Mountain Health Plans (RMHP) offers Disease Management Programs that can help you manage your chronic health conditions. Our programs offer learning materials, advice and care tips. We also help you work with your provider. The goal is to help you learn to manage your health conditions.
Members are found through medical or pharmacy claims. Members are enrolled in the program(s) that meets their needs. You will receive educational material by mail and you may receive a call from our Chronic Disease Health Educator to answer your questions and discuss your health care plan.
If your health condition is more complex we may have a Nurse Case Manager call you. This nurse will work with your provider to make sure you have what you need to stay well.
In addition to being enrolled through claims, you also can self-refer to the programs by calling 1-800-843-0719, ext 5661, or 970-255-5661. There are certain requirements that you must meet.
It is your choice to be in these programs. You can choose to be removed from a program at any time. If you do not want you or your child to take part in these programs, please call us at the number listed above.
