Follow three simple steps to fax, email, or mail your application!

1. Complete the RMHP Medicare Enrollment Application (Español) online or print it and complete it by hand.
2. Sign the application.  If you selected Electronic Funds Transfer for your payment option, be sure to sign that section, too.  All fields within the Payment Option section must be completed.
3. Submit the Application to Rocky Mountain Health Plans using your preferred method:
   
   
   1. Fax – 970-255-5666  A Fax Cover Sheet is included with the Enrollment Application.
   2. Email – Scan the document and include it as an email attachment to MedicareSalesGroup@rmhp.org
