RMHP has served Medicaid-eligible persons in Western Colorado since 1974. We work closely with the State of Colorado and with local private and government organizations to provide or coordinate the delivery of publicly funded Colorado State Medicaid benefits.
Rocky Mountain Health Plans serves Medicaid clients living in these counties:
- Rio Blanco
- San Miguel
RMHP arranges for Medicaid clients to receive benefits covered by the Colorado State Medicaid program. All providers in the RMHP network are available to Colorado State Medicaid clients. RMHP also provides customer service, case management, disease management, and quality monitoring and improvement services.
RMHP doctors give special check-ups and follow-up services to children through age 21 who are covered by the Medicaid EPSDT program. RMHP nurses work closely with Members who have diabetes, asthma, heart disease, and other chronic illnesses to help them manage their health status.
Colorado State Medicaid offers some additional services that are not covered through Rocky Mountain Health Plans. RMHP Customer Service representatives will direct you to the right places to get dental care, mental health care, and substance abuse treatment through the Special Connections program.

How to Join RMHP Medicaid
You must be eligible for Medicaid before you can enroll in RMHP Medicaid. If you want to find out about your Colorado Medicaid eligibility, contact your county department of human services.
If your department of human services says you are eligible, you may choose to enroll in RMHP Medicaid. To do this, call HealthColorado toll-free at 888-367-6557. Tell them you want to join Rocky Mountain Health Plans.

Medicaid Accountable Care Collaborative
The Accountable Care Collaborative (ACC) is a new Medicaid program designed to improve health and reduce costs. Medicaid Members in the ACC will receive the regular Medicaid benefit package, and will also belong to a "Regional Care Collaborative Organization" (RCCO).
RCCO connects Medicaid Members to Medicaid providers and also helps Medicaid Members find community and social services in their area. An RCCO can also help Medicaid Members get the right care when they are returning home from the hospital or a nursing facility. Medicaid Members can choose a Primary Care Medical Provider (PCMP). A PCMP is your main health care provider, serving as a “medical home,” where you will get most of your health care. When you need specialist care, the PCMP will help you find the right specialist.
