Vision Plans


A variety of vision benefit programs are available, such as discounts for eye exams, eyeglasses, contact lenses, and laser vision correction, through our partnership with Vision Service Plan. If your employer group health plan has 10 or more employees enrolled, click on the links below to learn more about specific vision benefit programs.

PDF Exam Plus $100 Plan: Vision exam covered with a $10 copay and a $100 allowance toward the purchase of glasses or contacts at a discounted price once every 12 months.

PDF Exam Plus Plan: Vision exam covered with a $10 copay and discounts on glasses and contact lenses once every 12 months.

PDF VSP Signature Plan: Vision exam covered with a $10 copay, lenses as part of a complete set of glasses are covered with a $10 copay along with $105 allowance toward the purchase of frames at a discounted price. Or $110 allowance for the cost of contacts and the fitting exam.


Dental Plans


We offer a variety of dental benefit programs through our partnership with Delta Dental Plan. These benefit programs offer coverage for routine cleanings, dental care, and orthodontia. Click on the links below to learn more about each specific dental benefit program.

PDF Delta Dental High Option 1000: $50 individual deductible with a maximum benefit of $1,000 per person per year. Preventive services covered at 100% not subject to the deductible and basic services covered at 80% after deductible.

PDF Delta Dental High Option 1500: $50 individual deductible with a maximum benefit of $1,500 per person per year. Preventive services covered at 100% not subject to the deductible and basic services covered at 80% after deductible.

PDF Delta Dental High Option Orthodontic Benefits: $50 individual deductible with a maximum benefit of $1,000 per person per year. Preventive services covered at 100% not subject to the deductible and basic services covered at 80% after deductible. Orthodontics (braces) covered at 50% for children.

PDF Delta Dental Low Option: $50 individual deductible with a maximum benefit of $1,000 per person per year. Preventive services covered at 100% not subject to the deductible and basic services covered at 50% after deductible.


Beta Health Association


A dental discount plan is available on an employer paid or voluntary basis through Beta Health Association. The plan is available to all sizes of employer groups. Discounts allow employees to save an average of 65% off of the dentist’s normal full fee. Learn more about the Beta Dental Plan.
