Employee Model Notices
COBRA Notice -- This notice will be sent to all individuals who had a COBRA qualifying event between September 1, 2008 to March 31, 2009, regardless of the type of qualifying event. This notice will be mailed on April 17, 2009.
COBRA Model Notice
Colorado Continuation of Coverage (CCOC) Notice - This notice will be sent April 17, 2009 to all individuals who have a qualifying event between September 1, 2008 and February 17, 2009 who elected to enroll in CCOC. This notice will also be sent to all individuals with a qualifying event from February 17, 2009 to March 31, 2009 who may or may not have elected to enroll in CCOC.
